There is no visible date to show when this document was completed, however it does make a reference to the Memorial Cross being given to Mrs. Margaret Elizabeth Mercer, the mother of Private Thomas Mercer. The Memorial Cross was only instituted by Parliamentary Order-in-Council 2374 on 01 December 1919, so it is a safe assumption that it was only sometime after that date that this form was completed. You will also note that Private Mercer was not elligible for other medals and awards as per the hand-written notation on the bottom left of the form.
